Sherman Creek Road begins at River Road (CR 249) just north of the border with Pennsylvania and runs east, parallel to the state line and just north it, as a two-lane undivided road.

CR 4 ends here at a bridge over the West Branch of the Delaware River to Delaware County. The road continues straight ahead under town jurisdiction for one block to end at the Quickway (NY 17/Future I-86).
