Middle Road is a major east/west route on the North Fork of Long Island. It runs parallel to Main Road (NY 25) about a mile to the north, but it is straighter and has more lanes and a higher speed limit. Therefore, it is generally a faster way to travel along the North Fork. Middle Road is a four-lane divided road from Mattituck to Southold and a two-lane undivided road from Southold to Greenport.

Middle Road has its western terminus at Sound Avenue, a small two-lane undivided road. It does not connect to any other county or state route at this end. There were plans to extend the Long Island Expressway (I-495) from Riverhead from Mattituck. Had this extension been built, it would have had its eastern terminus at the western terminus of CR 48.
