Lake Avenue begins at East Moriches-Riverhead Road (CR 51) and runs north as a two-lane undivided road.

Lake Avenue ends at a traffic circle at Riverleigh Avenue (CR 104), Flanders Road (NY 24), and Nugent Drive (NY 24/CR 94). CR 63 continues straight ahead as Peconic Avenue.

Peconic Avenue crosses a bridge over the Peconic River to end at Main Street (NY 25). Turn right to go east or left to go west.
