WV 9 is an east/west route through the Eastern Panhandle of West Virginia. Oldtown Road (MD 51) becomes WV 9 upon crossing a bridge over the Potomac River and entering Paw Paw, West Virginia. After passing through the town, WV 9 in known as Paw Paw Road. It runs south for a couple of miles, and then WV 9 turns east onto Capacon Road while Paw Paw Road continues south as WV 29. Capacon Road runs east for a short distance and then north to the Potomac River at Great Capacon. There, it curves to the east again and continues a short distance further to Berkeley Springs. There, Capacon Road ends at US 522.

WV 9 is multiplexed with US 522 for a few blocks to the south in Berkeley Springs and then splits off to the east onto Martinsburg Road. As it approaches Hedgesville, it changes name to become Hedgesville Road, which runs southeast toward Martinsburg. Coming into Martinsburg, there is an interchange with I-81 where Hedgesville Road becomes Edwin Miller Boulevard. After about a mile, US 11 joins WV 9 along Edwin Miller Boulevard, which becomes North Queen Street and then South Queen Street, running south through the city. WV 45 joins the two routes after a short distance, and then US 11 splits off. A short distance further, WV 9 and WV 45 both split off from South Queen Street at the same point, going east and west respectively.

This next portion of WV 9 is known as Charles Town Road. It quickly widens to become an expressway that continues southeast through the panhandle toward Charles Town, where it is joined by US 340. At the next exit, WV 9 splits off from the expressway and runs east to the Shenandoah River. After crossing a bridge over the river, WV 9 narrows down to two lanes as it enters Virginia and becomes Charles Town Road (VA 9).
